New Patient Forms

For your convenience, because we respect your time!

Here are a few basic forms to fill out for your first visit with Dr. Elsasser. We have included these forms here for your convenience. You can click on each form, fill it out and bring it with you to your first visit. The first 3 forms are what we need in the office for your records. The Insurance Verification Form is for your benefit. It is a helpful script for you to follow to verify your own insurance. It is beneficial for you to understand what your insurance intells. In addition, on your first visit we will need to see your insurance card, if filing insurance, and your driver's license or identification. The last form is for minors under the age of 19 only.

What to expect at your first visit to our office

You will first meet with our new patient coordinator to discuss the paper work and answer any question you may have.
A consultation with the doctor to fully explain you or your child’s problem.
A complete spinal examination to locate levels of subluxation that may be the cause of your problem.
Safe, low dose X-rays will be taken, if indicated, to determine complicating factors.
A complete review of findings will be scheduled. All available options will discussed at this time.
If yours is not a chiropractic case, referral to another professional will be made.
