Here are a few basic forms to fill out for your first visit with Dr. Elsasser.  We have included these forms here for your convenience.  You can click on each form, fill it out and bring it with you to your first visit. The first 3 forms are what we need in the office for your records. The Insurance Verification Form is for your benefit.  It is a helpful script for you to follow to verify your own insurance.  It is beneficial for you to understand what your insurance intells. In addition, on your first visit we will need to see your insurance card, if filing insurance, and your driver's license or identification. The last form is for minors under the age of 18 only.
